ORDERING INFORMATIONS

To ensure prompt processing, please provide:
1. Account name
2. Billing and shipping address
3. Purchase order number (if applicable)
4. Your name and telephone number
5. Product number / description
6. Unit of measure and quantity desired

 

Any incorrect information may delay delivery.  If any correction is made after the package is already in transit, there will be a correction charge of $10 PER PACKAGE.

Special Orders
Case quantity purchase may be required. Special orders CANNOT be cancelled or returned.
Prepayment is required.


Back Orders
We strive to deliver all orders complete the first time. In the event that the desired product is out of stock at the time of ordering, we may back order the item upon your request and ship it as soon as it becomes available, or ship from another distribution center if applicable. Appropriate freight charges will apply.
 

SHIPPING INFORMATION

Shipping Information

All orders will be delivered via FedEx Ground Service unless requested otherwise. Delivery by FedEx Service is available at actual shipping cost plus $3.00 handling charges.  Weekend delivery service is NOT available.   Orders received by 1:00pm will usually be shipped on the same day. Please do not use a P.O. Box address for UPS does not deliver to P.O. Box addresses.  For AK and HI residents USPS Priority Mail will be used instead.

There is an address correction charge of $10 PER PACKAGE for any incorrect address given.

In the event that the shipped out package is returned to us due to the fact that no receiver was available at the requested delivery destination, the purchaser is responsible for both the outgoing and returning shipping charges.

Terms and Conditions
PAYMENT INFORMATION
Payment: We accept personal or business checks, money orders, and most major credit cards (Visa, MasterCard, American Express, and Discover). Please make checks payable to K.S. Choi Corp. Only USD funds through a United States Financial Institute are accepted. Returned checks will be subjected to $30.00 penalty charge.

Payment Term: 1.5% per month late charge will be assessed on all past due invoices and no orders will be shipped against delinquent accounts.

Credit Card Purchases
We accept Visa, MasterCard, American Express, and Discover Charge Cards. Please have credit card number, card holder's name, expiration date and the security code ready when placing your order.
 
RETURN INFORMATION
1. We will ask for the invoice number and date of purchase, item number, quantity and reason for return, and provide you with a Return Authorization Number.

2. Write the Return Authorization Number on your shipping label. Return the item prepaid via UPS, in its original packaging and original condition to:

All returns require a receipt with Return Authorization Number to be processed. All merchandise returns are subject to inspection. Any merchandise unsuitable for resale will not be accepted. A minimum restocking fee of 25% will be applied to all returns. We will not accept returns after 30 days from the date of purchase. Books must be returned within 7days from the date of the purchase. For defective products we will replace or repair based on manufacturer's policy. 

When returning a merchandise which was initially offered a free shipping, the amount to be refunded will be the price of the returned merchandise less the initial shipping charge.

FDA COMPLIANCE PURCHASE AND SAMPLE REQUESTER AGREEMENT
 

The use of acupuncture needles is restricted to qualified practitioners as determined by each state. With this restriction, K.S. Choi Corp. will not offer any medical advice or make any medical claims on the offered acupuncture needles. In compliance with the FDA, and State regulations that restrict the use of acupuncture needles to qualified practitioners, K.S. Choi Corp. will sell acupuncture needles and devices to licensed professionals only. All acupuncture devices are sold to qualified practitioners only. The purchaser accepts full responsibility for the safe and proper use of these devices and agrees to indemnify and hold K.S. Choi Corp. harmless from any consumer claims against the ultimate use of these acupuncture devices.
