To ensure prompt processing, please provide:
1. Account name
2. Billing and shipping address
3. Purchase order number (if applicable)

6. Unit of measure and quantity desired
Special Orders
Case quantity purchase may required. Special orders CANNOT be cancelled or
returned.
Back Orders
We strive to deliver all orders complete the first time. If for some reason
a product is out of stock at the time of ordering, we can back order the
item and ship it as soon as it is received, or ship from another
distribution center if available. Appropriate freight charges will apply.

| SHIPPING INFORMATION |
Shipping Information
Delivery by UPS Service is available at actual shipping cost plus $3.00
handling charges. (A $3.00 handling charge will not be added to all orders
over $100.00 net) Orders will be delivered via UPS Ground Service. All
orders received by 3:00pm will be shipped on the same day. Do not use a P.O.
Box address. UPS does not deliver to P.O. Box addresses.

Prices in the catalogue can be change anytime and the tax, shipping charges
are not included in these listed prices.

| PAYMENT INFORMATION |
Payment: We accept personal or
business checks, money orders, and Visa, MasterCard, American Express, or
Discover Charge cards. Checks payable in US funds to K.S. Choi Corp.
Returned checks will be subjected to $30.00 penalty charge.
Payment Term: 1.5% per month late charge will be assessed on all past
due invoices and no orders will be shipped against delinquent accounts.
Open Accounts
For your convenience, K.S. Choi Corp offers open account purchases. These
accounts are subject to our open account terms and satisfactory credit

Credit Card Purchases
We accept Visa, MasterCard, American Express, and Discover Charge Cards.
Please have credit card number, card holder's name and expiration date ready
when placing your order.

| RETURN INFORMATION |
1. We will ask for the invoice number and
date of purchase, item number, quantity and reason for return, and provide
you with a Return Authorization Number.
2. Write the Return Authorization Number on your shipping label. Return the
item prepaid via UPS, in its original packaging and original condition to:

You must have a Return Authorization Number to return an item. All
merchandise returns are subject to inspection. Any merchandise unsuitable
for resale will not be accepted. A minimum restocking fee of 15% will be
applied to all returns over 30 days from date of purchase. Additional
restocking fees are charged on any return which exceeds 60 days from date of
purchase. We will replace or repair defective items based on manufacturer's
policy. Books must be returned within 7days of receipt with a authorization. |

In compliance with the FDA, and State regulations that restrict the use of acupuncture needles to qualified preactitioners,
KS Choi will sell acupuncture needles and devices to licensed professionals only. All acupunture devices are sold to qualified
practitioners only. The purchaser accepts full responsibility for the safe and proper use of these devices and agrees to
indemnify and hold KS Choi Inc. harmless from any consumer claims against the ultimate use of these acupuncture devices.
